What is the Prevention Score?
The Prevention Score is a dynamic tool to help you track your progress, over time, in the health risks that impact your health status. It is not a measurement of your health status…just a way to track your prevention efforts.
HOW IS MY SCORE DETERMINED?
There are two key components:
Assessment - based on the results of your blood test, biometric screening and Health Appraisal; worth up to 500 points. (You must complete a second assessment at the end of the current membership year to receive a Final Assessment Score, which will be used to calculate your Final Prevention Score.)
Intervention - based on the actions you take throughout the year to improve your health, such as completing recommended screenings and online Action Programs, activity tracking, participating in Community Events and online Challenges; worth up to 500 points.
